Includes everything in Essential Oversight, plus proactive coordination across providers, service planning, problem solving, and regular, personalised communication with care teams and family. Our approach emphasises early, skilled intervention to preserve independence, manage setbacks and support clients to continue living at home on their own terms.

For clients with significant medical complexity, progressive illness or rapidly changing needs. Includes everything in Comprehensive Care Coordination plus close clinical oversight, coordination with multiple specialist treating teams, support through transitions and crises, comprehensive family communication and support
