Specialist Companions

Each Specialist Companion works with a small number of clients to support day-to-day life, noticing changes and contributing to continuity over time.

The work is consistent, relationship-based, and requires attention, judgement, and follow-through. You will work with the same people over time and are expected to notice, think, and act - rather than simply complete tasks.

  • You will be trusted to:

    • Take responsibility for your work without being prompted

    • Notice small changes early and act on them, with support from the Care Coordinator

    • Communicate clearly, directly, and in a timely manner, adapting your approach to each client and circumstance

    • Work within clear boundaries while maintaining a relational, person-centered approach

    • Use judgement, including knowing when to ask for support

    • Follow through consistently and communicate any missed care or issues.

Care Coordinators

The central point of coordination across each client’s care.

Responsible for maintaining consistency and continuity across clients and staff, ensuring issues are identified early, decisions are made promptly, and the right actions are taken to keep care on track.

This is not a high-volume role. We work with a small number of clients and expect a level of reliability, discretion, and judgement that reflects that.

  • You will be trusted to:

    • Maintain clear oversight across multiple clients and staff

    • Accurately interpret information from different sources

    • Identify emerging issues early and ensure an appropriate response

    • Communicate effectively with clients, families, and team members

    • Support and guide staff in real time

    • Make decisions within scope, and escalate appropriately

    • Hold clear boundaries and standards across the service while supporting a relational approach to care

    • Maintain accurate, current information and ensure it is shared appropriately and clearly across the team and clients
